The Senior/Lead Pastor is responsible for submitting the following:

1. Official Church of the Nazarene Report (click here)
This is the official denominational report completed online each year by the Lead Pastor. It includes attendance, membership, financial information, and a summary of ministry activity.
2. Pastor’s Video Report
A short video update highlighting ministry, key moments, and celebrations from the past year.
Please note this form requires a Google email address to submit. If you do not have a Google email address please email us for support.
3. Church Directory
Reporting your church’s and pastor’s contact information for the district office records and for the District Journal.
4. Memorial Report
Names of those from your congregation who have passed away during the past church year. They will be honoured at District Assembly.
5. Certificate of Election
Completed following your local church elections.
6. NazSafe & Insurance Document
Submit your completed NazSafe compliance, along with proof of current insurance (combined into one submission)
7. Staff & Ministry Role Form
This form is to be completed before a church hires a new staff member (paid or unpaid).
Renewal is required each year along with District Assembly forms.
All Associate Pastors, District Licensed Pastors, Chaplains and those serving in SPC (Specialized Ministry) are required to complete.
8. Annual Report of Ordained or District Licensed Pastors
· This report must be completed individually.
· It applies to all credentialed ministers, including those serving in specialized or non- local church roles.
9. Application/Renewal Form for Pastors
This form is for pastors who are applying for first time or renewal Special Service (SPC) status.
